Endometriosis is a medical condition where tissue similar to the lining of the uterus, called endometrium, grows outside the uterus. This tissue can be found on the ovaries, fallopian tubes, and other pelvic organs. During the menstrual cycle, the endometrial tissue outside the uterus also thickens, breaks down, and bleeds. However, since this blood has no way to exit the body, it can cause inflammation, pain, and the formation of scar tissue or adhesions.

The exact cause of endometriosis is not fully understood, but it is believed to involve a combination of genetic, hormonal, and immune factors.

Endometriosis can have various effects on fertility:

Distorted pelvic anatomy: The growth of endometrial tissue outside the uterus can lead to the formation of adhesions or scar tissue. This can distort the normal anatomy of the reproductive organs, making it difficult for the egg to travel from the ovary to the fallopian tube or for the sperm to reach the egg.

Inflammation: Endometriosis causes chronic inflammation in the pelvic cavity, which can negatively impact the function of the ovaries, fallopian tubes, and uterus.

Alterations in the pelvic environment: The presence of endometrial implants may create an unfavourable environment for conception and implantation.

Changes in the immune system: Endometriosis may affect the immune system, making it less effective in recognizing and clearing abnormal cells, including endometrial cells growing outside the uterus.

Treatment options for endometriosis include:

Pain medication: Over-the-counter pain relievers or prescription medications can help manage pain associated with endometriosis.

Hormonal therapies: Hormonal treatments, such as birth control pills, progestin therapy, or GnRH agonists, aim to suppress the growth and activity of endometrial tissue.

Surgery: In more severe cases or when fertility is a concern, surgical interventions may be recommended. This can involve the removal of endometriosis implants, adhesions, or, in some cases, a partial or total hysterectomy.

Assisted reproductive technologies (ART): In cases where natural conception is challenging, assisted reproductive technologies such as in vitro fertilization (IVF) may be used to help achieve pregnancy.

It’s important for individuals experiencing symptoms of endometriosis, especially if fertility is a concern, to consult with a healthcare professional for a proper diagnosis and to discuss appropriate treatment options tailored to their specific situation.
